บทคัดย่อ

นโยบายของกระทรวงสาธารณสุขที่เปลี่ยนแปลงระบบการเงินโรงพยาบาลภาครัฐเข้าสู่ระบบกองทุนหลักประกันสุขภาพแห่งชาติในปี2545  ทำให้โรงพยาบาลจำเป็นต้องให้ความสำคัญกับการจัดการต้นทุนการรักษาพยาบาล การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาต้นทุนการรักษาพยาบาลผู้ป่วยฟันผุของโรงพยาบาลทันตกรรม คณะทันตแพทยศาสตร์เปรียบเทียบกับรายรับที่ได้รับการชดเชยจากสำนักงานประกันสุขภาพแห่งชาติ (สปสช.) ปีงบประมาณ 2556 ของผู้ป่วยที่ใช้สิทธิหลักประกันสุขภาพ เป็นการศึกษาเชิงปริมาณ โดยการเก็บข้อมูลย้อนหลังจากแหล่งข้อมูลทุติยภูมิ ใช้เกณฑ์ในการกระจายต้นทุนโดยวิธีจากบนลงล่าง ผลการศึกษาพบว่า ต้นทุนที่เกิดจากรักษาพยาบาลของผู้ป่วย 1,201 ราย มีจำนวน 2,789,279.72 บาทในขณะเดียวกัน มีรายรับที่ได้จากการชดเชยจากโรงพยาบาลศรีนครินทร์  คณะแพทยศาสตร์  มหาวิทยาลัยขอนแก่นเพียง  1,442,789.50  บาท (ร้อยละ 51.73 ของต้นทุนรวมในการรักษา) ซึ่งไม่เพียงพอกับค่าใช้จ่ายที่เกิดขึ้น จึงได้เสนอแนวทางในการบริหารจัดการค่ารักษาพยาบาลโดยการเสนอแนวทางการปฏิบัติที่ดี การกำหนดต้นทุนมาตรฐานแต่ละขั้นตอน การกำหนดขั้นตอนการทำงาน การลดกิจกรรมที่ไม่ได้วางแผน การใช้เทคโนโลยีช่วยในการทำงาน และการจัดทำแนวการปฏิบัติมาตรฐานในการใช้วัสดุ ยาและเวชภัณฑ์ เพื่อนำไปสู่การเพิ่มผลลัพธ์ที่มีประสิทธิภาพและการลดต้นทุนการใช้ทรัพยากร

ABSTRACT

According of the changing of finance policy of Ministry of Public Health, all hospitals have to emphasize in cost management. The objectives of this research were to investigate the treatment cost of caries patients who paid by National Health Security Office (NHSO) in the fiscal year 2013 of Dental Hospital, Faculty of Dentistry, Khon Kaen University. Comparison of the treatment cost and the subsidized incomes were analyzed. The retrospective study were obtained from secondary data then all expenses were collected and grouping while the cost allocation were classified by top down approach. The results shown that during 1^st October 2012 to 30^th September 2013 there were 1,201 NHSO patients were cured with total cost of 2,789,279.72 Baht. However, the subsidized incomes from NHSOwas only 1,442,789.50 Baht (51.73 % of total cost treatment). For insufficient fund problems, the suggestions were proposed including : the good practices, fixed cost of each step, method statement, reduce unplanned activities, technology using and standard of drugs and medical supplies for increasing the efficiency and also reducing  resource usage.
