บทคัดย่อ

การกระจายอำนาจด้านสุขภาพไม่ได้เป็นเพียงการถ่ายโอนงบประมาณ บุคลากร อาคารสถานที่ และภารกิจการให้บริการไปยังหน่วยการปกครองท้องถิ่น บทความนี้ใช้แนวคิดขีดความสามารถเชิงนโยบาย (Policy Capacity) ของ Wu, Ramesh และ Howlett และแนวคิดพื้นที่การตัดสินใจ (Decision Space) ของ Bossert ในการวิเคราะห์หน้าที่และอำนาจด้านสุขภาพปฐมภูมิที่ได้รับการถ่ายโอนให้แก่องค์กรปกครองส่วนท้องถิ่น โดยวิเคราะห์เอกสารเชิงเปรียบเทียบอย่างเป็นระบบครอบคลุมกรณีศึกษา 8 ประเทศตามแบบแผนการเปรียบเทียบกรณีที่แตกต่างกันมากที่สุด (Most Different Systems Design: MDSD) ได้แก่ โปรตุเกส รัสเซีย ญี่ปุ่น ฟิลิปปินส์ อินโดนีเซีย เม็กซิโก บราซิล และชิลี โดยดำเนินการตามระเบียบวิธีการวิเคราะห์เอกสาร (READ Framework) และหลักความโปร่งใสของ PRISMA 2020 และ PRISMA-S ผลการศึกษาพบว่า ขีดความสามารถเชิงปฏิบัติการในระดับระบบ (Systemic Operational Capacity) คือ คอขวดที่ปรากฏซ้ำมากที่สุดในกระบวนการกระจายอำนาจด้านสุขภาพปฐมภูมิใน 8 ประเทศ ได้แก่ ระบบเงินอุดหนุนให้แก่องค์กรปกครองส่วนท้องถิ่น ระบบการเตรียมความพร้อมกำลังคน ยา และเวชภัณฑ์ ระบบส่งต่อ การประสานงานระหว่างหน่วยบริการต่างสังกัด และการบังคับใช้มาตรฐานบริการสุขภาพ ในขณะที่ขีดความสามารถเชิงวิเคราะห์ (Analytical Capacity) ในทุกระดับมีบทบาทสำคัญสำหรับกระบวนการกระจายอำนาจ ได้แก่ ขีดความสามารถในการวิเคราะห์ข้อมูล การวางแผน และการเรียนรู้เชิงนโยบายของทุกองคาพยพในระบบสุขภาพในระดับพื้นที่ ในขณะที่ขีดความสามารถเชิงการเมือง (Political Capacity) ช่วยสร้างความชอบธรรมและการอภิบาลระบบ (Stewardship) ในระยะเปลี่ยนผ่านของการกระจายอำนาจ นอกจากนี้ ในบรรดามิติของขอบเขตอำนาจตัดสินใจ (Decision Space) กลไกกำกับดูแลมาตรฐานบริการสุขภาพปฐมภูมิเป็นปัจจัยความสำเร็จของการกระจายอำนาจด้านสุขภาพปฐมภูมิในประเทศกรณีศึกษา 8 ประเทศ โดยมีเงื่อนไขสำคัญคือ ระบบเงินอุดหนุนที่รัฐบาลจัดสรรให้แก่องค์กรปกครองส่วนท้องถิ่นและระบบหลักประกันสุขภาพถ้วนหน้าของภาครัฐที่ช่วยสร้างตาข่ายคุ้มครองทางสังคมให้แก่กลุ่มเปราะบางในระยะเปลี่ยนผ่าน
