การพัฒนาระบบบริการสุขภาพชุมชน: ด้านการส่งเสริมสุขภาพแม่และเด็ก ในพื้นที่เทศบาลเมืองบุรีรัมย์ จังหวัดบุรีรัมย์

บทคัดย่อ
จากแผนงานด้านการควบคุมโรคและป้องกันสุขภาพประเทศไทย พ.ศ. 2562-2564 ได้กำหนดให้มีการดำเนินงานและสร้างกิจกรรมส่งเสริมสุขภาพของกลุ่มแม่และเด็ก ต้องการให้ได้รับบริการสุขภาพเป็นไปตามมาตรฐานสาธารณสุข ซึ่งเทศบาลเมืองบุรีรัมย์ได้นำแผนงานและหลักการในการดูแลสุขภาพมาประยุกต์ใช้ปฏิบัติงานภายในองค์กรของหน่วยงานสาธารณสุข การวิจัยนี้จึงต้องการพัฒนาระบบการดูแลสุขภาพของแม่และเด็กในพื้นที่เทศบาลเมืองบุรีรัมย์ จังหวัดบุรีรัมย์ที่มีประสิทธิภาพสูง พร้อมทั้งสร้างให้เกิดความพึงพอใจในแม่และเด็กที่เพิ่มสูงขึ้น การวิจัยมีวัตถุประสงค์เพื่อเปรียบเทียบระบบการให้บริการสุขภาพแก่แม่และเด็กแบบใหม่และแบบเก่า และประยุกต์ใช้กระบวนการดูแลโดยยึดผู้ป่วยเป็นศูนย์กลางร่วมกับแนวทางการพัฒนาระบบแบบคุณภาพพื้นฐาน 3P ทดลองในกลุ่มประชากร คือ ผู้ปกครองของเด็กที่รับบริการภายในศูนย์แพทย์ชุมชนโรงพยาบาลบุรีรัมย์ สาขา 2 จำนวน 59 คน และกลุ่มตัวอย่าง คือ กลุ่มแม่และเด็กที่ประสงค์เข้าร่วมกิจกรรมภายในศูนย์แพทย์ชุมชนโรงพยาบาลบุรีรัมย์ สาขา 2 จำนวน 51 คน ในพื้นที่ 18 ชุมชนเขตเทศบาลเมือง มีการประยุกต์ใช้เครื่องมือเป็นแบบสอบถามแบบมีโครงสร้าง 5 ระดับ และมีค่าสัมประสิทธิ์แอลฟาครอนบาค ที่ 0.91 และวิเคราะห์เปรียบเทียบโดยสถิติ t-test Independent พบว่า ระบบการดูแลสุขภาพแม่และเด็กที่พัฒนาใหม่มีระดับคะแนนประเมินประสิทธิภาพสูงกว่ารูปแบบเดิม อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 และปรากฏความสัมพันธ์กันของระดับคะแนนประเมินประสิทธิภาพที่เป็นไปในทิศทางตรงกันข้ามกัน [r]=-0.087 ซึ่งผลลัพธ์การวิจัยที่เกิดขึ้นนั้น กระตุ้นให้เกิดการบูรณาการร่วมกันของหน่วยงานที่มีความเกี่ยวข้องกับการให้บริการสุขภาพ จนสามารถสร้างให้เกิดความพึงพอใจของผู้ใช้บริการสุขภาพที่พัฒนาในระดับมากที่สุด (=4.47, S.D.=0.238) ซึ่งระดับความพึงพอใจมีความสัมพันธ์กับระดับความสุขที่เกิดขึ้นในขณะที่ได้รับการบริการสุขภาพและมีความสัมพันธ์ไปในทิศทางเชิงบวก [r]=0.238 โดยประโยชน์ที่ได้รับจากการวิจัยครั้งนี้ทำให้เกิดระบบการให้บริการสุขภาพแก่แม่และเด็กที่มีประสิทธิภาพสูงกว่าระบบแบบเดิม ซึ่งสอดคล้องตามพันธกิจหลัก 3 ด้านของเทศบาลเมืองบุรีรัมย์ โดยผลลัพธ์นั้นช่วยให้เกิดความเชื่อมั่นในระบบให้บริการ พร้อมช่วยส่งเสริมสุขภาพที่ดีให้กับกลุ่มแม่และเด็ก โดยเด็กนั้นจะปรากฏพัฒนาการที่สมวัยพร้อมทั้งได้รับวัคซีนตามเกณฑ์ที่กำหนด ทั้งยังสร้างระบบดูแลสุขภาพตามสิทธิประโยชน์ของประชาชนที่ควรจะได้รับบริการสุขภาพที่มีประสิทธิภาพสูงจากภาครัฐ ซึ่งการวิจัยนี้เป็นกรณีศึกษาเฉพาะพื้นที่ให้บริการสุขภาพแม่และเด็กในเทศบาลเมืองบุรีรัมย์ ซึ่งสามารถนำกระบวนการพัฒนาในครั้งนี้ไปประยุกต์ใช้ในการให้บริการสุขภาพแก่แม่และเด็กให้มีประสิทธิภาพสอดคล้องกับความต้องการในการดำรงชีวิตและบริบทสังคมในยุคสมัยปัจจุบันได้
