การศึกษาการให้บริการการแพทย์แผนไทยของสถานบริการภาครัฐ ในจังหวัดเชียงใหม่

บทคัดย่อ

การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อศึกษาสภาพ, กระบวนการการให้บริการ และประสิทธิภาพการให้บริการการแพทย์แผนไทย ของสถานบริการภาครัฐในจังหวัดเชียงใหม่ โดยใช้วิธีศึกษาแบบผสานวิธี (Mixed methodology) ซึ่งประกอบไปด้วยการวิจัยเชิงคุณภาพ และการวิจัยเชิงปริมาณ กลุ่มตัวอย่างของการวิจัยเชิงคุณภาพครั้งนี้ได้แก่ผู้บริหารและบุคลากรผู้ให้บริการการแพทย์แผนไทยรวมทั้งสิ้น 56 คน ส่วนการวิจัยเชิงปริมาณจะเป็นจำนวนของผู้มาใช้บริการจำนวน 657 คนในสถานประกอบการจำนวน 4 แห่งในจังหวัดเชียงใหม่ซึ่งประกอบด้วย โรงพยาบาลประสาทเชียงใหม่ โรงพยาบาลสารภี โรงพยาบาลแม่ออน และโรงพยาบาลสันทราย รวมถึงหน่วยบริการย่อยของสถานประกอบการดังกล่าวโดยเครื่องมือที่ใช้ในการศึกษาครั้งนี้ประกอบด้วย แบบสำรวจภาคสนาม แบบสังเกต แบบสัมภาษณ์เชิงลึก และแบบสอบถาม

ผลการศึกษาพบว่า สภาพสถานที่และกระบวนการให้บริการให้บริการการแพทย์แผนไทยของสถานประกอบการแทบทุกแห่งอยู่ในระดับมาตรฐานในการให้บริการ มีการซักประวัติ การสอบถามอาการเจ็บป่วย การรักษาด้วยการนวดแผนไทย ตามด้วยการประคบสมุนไพร และการให้คำแนะนำในการดูแลสุขภาพหลังจากการรักษา เพื่อเป็นการป้องกันโรค และอาการเจ็บปวดไม่ให้กลับมาอีก นอกจากนี้พบว่าผู้มารับบริการรักษาด้วยการแพทย์แผนไทยของสถานประกอบการ จำนวน 657 คน ส่วนใหญ่เป็นเพศหญิงที่สมรสแล้ว ช่วงอายุ 61-70 ปี มารับบริการรักษาอาการเจ็บป่วย เช่น ปวดศีรษะ คอ บ่า ไหล่ แขน และหลัง มีค่าใช้จ่ายในการบริการประมาณ 201-300 บาทต่อครั้ง หลังจากที่ใช้บริการการแพทย์แผนไทยแล้ว ส่วนมากจะมีอาการดีขึ้น และบางรายจะกลับมาใช้บริการอีก

The Study of Thai Traditional Medical Service by Public Hospitals in Chiang Mai Province

The objectives of this study were to find out nature of Thai traditional medicine, services provided by public hospitals in Chiang Mai province (Chiang Mai Neurological Hospital, Saraphi Hospital, Mae-on Hospital, Sansai Hospital included their partner centers), and their efficiency in providing the services by Mixed methodology that included Qualitative and quantitative research. The samples of this research composed of executives and Thai Traditional medicine service providers for the total of 56 people in qualitative’s. For the qualitative research, 657 service users were conducted. Tools used were field questionnaires, observative questionnaires, In-depth Interview, and user’s questionnaires.

The results revealed that the service sites reached the specified standard. Service procedures consisted of inquiring about patients’ personal information, inquiring about their symptoms of sickness both before and after providing Thai traditional massage services, using herb balls, and giving advice on health care. Most of all hospitals/ health care centers, i.e. Chiangmai Neurological Hospital , Maeon Hospital, Sansai Hospital, Wat Tonhiew health care center, and wat Hueykiang health care center were found to follow the service procedures previously stated. 657 patients who received the services from those four hospitals/health care centers were married-female, 61-70 years old. Before they came to the centers, headaches, neckaches, shoulderaches, armaches, and backaches were their symptoms. The expense for each service was 201-300 baht. Most of them recovered from their symptoms and some wanted to use the services again.
