The objective of this research were 1) to study problem situations. The target groups in the research were 25 people which was obtained by means of a specific selection the research instruments consisted of record, document analysis, questionnaire, in-depth interview. Stakeholder Meeting Record form data analysis use content analysis and summarize the main points, classify, compile lists by analyzing content (Content Analysis) by merging information issues of stakeholder meetings. The results of the research found that 1) the cause of the accident was caused by people, cars, road conditions and the environment.
