บทคัดย่อ

การวิจัย ยุทธศาสตร์การจัดการระบบสุขภาพชุมชนอย่างยั่งยืน จังหวัดน่าน เป็นการวิจัยและพัฒนา (Research and Development : R&D) มีวัตถุประสงค์เพื่อพัฒนายุทธศาสตร์การจัดการระบบสุขภาพชุมชนอย่างยั่งยืนที่มีความเหมาะสมและสอดคล้องกับบริบทของจังหวัดน่าน เป็นการวิจัยเชิงคุณภาพที่นำกระบวนการของวัฏจักรนโยบายมาใช้ในการกำหนดขั้นตอนในการศึกษาวิจัย และใช้แผนที่ทางเดินยุทธศาสตร์เป็นเครื่องมือในการพัฒนาแบบจำลองเชิงทฤษฎีขององค์ประกอบที่มีอิทธิพลต่อการจัดการระบบสุขภาพชุมชน นำไปสู่การขับเคลื่อนเป็นนโยบายสาธารณะในการจัดการระบบสุขภาพชุมชนอย่างยั่งยืน จังหวัดน่าน กลุ่มผู้ให้ข้อมูลสำคัญได้แก่ บุคลากรทางด้านสาธารณสุขในระดับจังหวัด อำเภอ ตำบล และผู้นำชุมชน ผู้นำท้องถิ่น อาสาสมัครสาธารณสุขและแกนนำสุขภาพชุมชนโดยกำหนดขั้นตอนของการวิจัย เป็น 6 ขั้นตอน ได้แก่ 1) การพัฒนาแบบจำลองเชิงทฤษฎีขององค์ประกอบที่มีอิทธิพลต่อการจัดการระบบสุขภาพชุมชน 2) การตรวจสอบความสอดคล้องกลมกลืนของแบบจำลองเชิงทฤษฎีขององค์ประกอบที่มีอิทธิพลต่อการจัดการระบบสุขภาพชุมชนกับข้อมูลเชิงประจักษ์ 3) การนำรูปแบบการจัดการระบบสุขภาพชุมชนไปทดลองใช้ในพื้นที่ 4) การประเมินผลการใช้รูปแบบการจัดการระบบสุขภาพชุมชนในพื้นที่ทดลอง 5) การปรับปรุงรูปแบบการจัดการระบบสุขภาพชุมชน และ 6) การขยายผลรูปแบบการจัดการระบบสุขภาพชุมชน โดยสรุปผลการศึกษาวิจัย ดังนี้

องค์ประกอบของการจัดการระบบสุขภาพชุมชนอย่างยั่งยืน ที่เหมาะสมและสอดคล้องกับบริบทของพื้นที่จังหวัดน่าน ตามแผนที่ทางเดินยุทธศาสตร์ทั้ง 4 ระดับได้แก่ 1) ระดับประชาชน ได้แก่ ชุมชนมีแผนสุขภาพของชุมชน ชุมชนจัดการสุขภาพตามบริบทชุมชน ชุมชนมีมาตรการทางสังคม ชุมชนมีระบบการเฝ้าระวังสุขภาพ และชุมชนมีวิถีชีวิตตามแนวเศรษฐกิจพอเพียง 2) ระดับภาคีเครือข่าย ได้แก่ องค์กรภาคีเครือข่ายที่มีส่วนร่วมในการจัดการสุขภาพชุมชน การสนับสนุนและการเสริมสร้างความเข้มแข็งให้แก่นักจัดการสุขภาพชุมชน และมีระบบการบริหารจัดการภาคีเครือข่ายเชื่อมโยงกันอย่างมีประสิทธิภาพ 3) ระดับกระบวนการ ได้แก่ การสร้างและขับเคลื่อนนโยบายสาธารณะ การถ่ายทอดความรู้และสื่อสารด้านสุขภาพ การจัดการความรู้และนวัตกรรมสุขภาพชุมชน มีการสร้างแรงจูงใจ และการติดตามประเมินผลการจัดการสุขภาพชุมชน 4) ระดับรากฐาน ได้แก่ ระบบข้อมูลสุขภาพชุมชนที่มีประสิทธิภาพ บุคลากรด้านสุขภาพในชุมชนมีคุณภาพ ชุมชนมีส่วนร่วมในการจัดการสุขภาพ การใช้เทคโนโลยีและนวัตกรรมที่เหมาะสมกับชุมชน การจัดการด้านทุนเพื่อสุขภาพพอเพียง ทุนทางสิ่งแวดล้อมและทุนทางวัฒนธรรมที่เอื้อต่อการจัดการสุขภาพ

ยุทธศาสตร์การจัดการระบบสุขภาพชุมชนอย่างยั่งยืน จังหวัดน่าน พบว่ามีปัจจัยด้านคน ทุน และความรู้เป็นปัจจัยพื้นฐานที่จะนำไปสู่การจัดกระบวนเรียนรู้ทางด้านสุขภาพในชุมชน โดยใช้แผนที่ทางเดินยุทธ์ศาสตร์เป็นเครื่องมือในการดำเนินงาน โดยประชาชนเข้ามามีส่วนร่วมในการดำเนินงานและภาคีเครือข่ายให้การสนับสนุน มีการประยุกต์ใช้ ภูมิปัญญาท้องถิ่นเข้ามาหนุนเสริม การจัดการระบบสุขภาพชุมชนภายใต้องค์ประกอบทางด้านทีมสุขภาพ ซึ่งมีนักจัดการสุขภาพชุมชนเป็นกลไกสำคัญในการขับเคลื่อนงานในชุมชน การมีทรัพยากรและทุนในการบริหารจัดการ การเรียนรู้และการสร้างนวัตกรรมสุขภาพชุมชน การมีแผนสุขภาพชุมชนที่ดี มีภาคีเครือข่ายและการขยายเครือข่ายร่วมกัน มีการกำหนดเป้าหมายผลการพัฒนาสุขภาพในระดับชุมชน ระดับครอบครัว และระดับบุคคล ซึ่งจะนำไปสู่ความสำเร็จ ความยั่งยืนและการขยายผลการจัดการระบบสุขภาพชุมชน ที่จะนำไปสู่การจัดการระบบสุขภาพชุมชนอย่างยั่งยืนต่อไป

The Strategy of Sustainable Community Health Management System in Nan Province.

This research and development approach (R&D) study aimed at developing strategies for sustainable community health management system in Nan Province. This qualitative research employed policy cycle process in formulating the research procedure and also incorporated strategic route map as a means to develop simulation based on compositional theory that influenced the acquisition of community health management system and thus led to the public policy in driving the sustainable community health management system in Nan Province. Key informants included public health officers at the provincial, district, and sub-district levels as well as community leaders, public health volunteers, and community health leaders. Research procedure was divided into six steps as follows: 1) Development of simulation based on compositional theory that influenced the acquisition of community health management system; 2) Goodness of fit index validation of the simulation; 3) Trying-out the developed community health management system; 4) Evaluating the implementation of the developed community health management system; and 6) Expanding the implementation of the developed community health management system to the other areas.

The findings showed that: The components for sustainable community health management system concurring with the situational context of Nan Province and in accordance with strategic route map included four levels were as follows. 1) At the people level, there were community held the community health management plan, community managed health programs in line with the situational context, community held social preventive measures, and community held health monitoring system; and community lived their lives based on self-sufficiency economy principles. 2) At the networking level, there were networked organizations participated in community health management as well as supported and strengthened the capacity of community health management specialists and held the system to effectively manage the networking. 3) At the procedural level, there were development and implementation of public policy, knowledge transfer and communication about health, knowledge management and innovation in community health, motivation building, and monitoring and evaluating community health management system. 4) At the foundation level, there were effective community health database, qualified health personnel in the community, community participation in health management, usage of suitable technology and innovation for the community, and management of sufficient health capital, environmental capital and cultural capital for health management.

Regarding strategies for sustainable community health management system in Nan Province, the findings revealed the foundation factors including people, capital, and knowledge that led to the learning process about health in the community by incorporating strategic route map as driving mechanism. The people took part and the community health network and supported the implementation of health management system. The local wisdom was integrated as scaffolding mechanism in community health management system following the health team approach as the community health specialists served as vital part in driving the community health programs. There was the learning and creation of innovation in community health management. There were practical plan in community health management, working network and expansion of the network, and targeted goals in developing health management at the community, family, and individual levels which could lead to success, sustainability, and expansion of community health management system.
