บทคัดย่อ
สืบเนื่องจากรัฐบาลได้ถ่ายโอน รพ.สต. จำนวน 3,263 แห่ง จากกระทรวงสาธารณสุขไปให้แก่ อบจ. 49 จังหวัด ในการนี้จึงทำให้เกิดข้อสงสัยว่า การถ่ายโอนดังกล่าวก่อให้เกิดผลกระทบต่อบริการที่ รพ.สต. รับผิดชอบอยู่หรือไม่ ดังนั้น การวิจัยนี้มีวัตถุประสงค์เพื่อประเมินผลทางด้านบริการของ รพ.สต. หลังจากการถ่ายโอน โดยหาคำตอบว่า (1) หลังการถ่ายโอนบริการที่ส่งมอบแก่ประชาชนดีขึ้นหรือแย่ลงกว่าเดิม (2) จุดอ่อน/จุดแข็งที่สำคัญของการบริการหลังการถ่ายโอนเป็นอย่างไร เมื่อเปรียบเทียบกับก่อนการถ่ายโอน (3) เพื่อเสนอแนะแนวทางปรับปรุงบริการของ รพ.สต. ให้มีสมรรถนะในการบริการเพิ่มขึ้น สำหรับกรอบแนวคิดในการวิจัยได้ใช้ตัวแบบ รพ.สต. ติดดาว เป็นกรอบเนื้อหาในการประเมิน ส่วนวิธีวิทยาการวิจัยได้ใช้วิธีวิจัยเชิงคุณภาพ เก็บรวบรวมข้อมูลแบบสามเส้าจากตัวอย่าง 32 รพ.สต. จาก 8 จังหวัด 4 ภาค ข้อมูลที่ได้นำมาวิเคราะห์และตีความโดยโปรแกรม ATLAS.ti ผลการวิจัยพบว่า ร้อยละ 60 ของบริการหลังการถ่ายโอน ยังคงเหมือนก่อนการถ่ายโอน ส่วนอีกร้อยละ 25 มีแนวโน้มดีกว่าเดิม เนื่องจากได้รับงบประมาณและบุคลากรเพิ่มขึ้น และที่เหลือประมาณร้อยละ 10 เห็นว่าแย่กว่าเดิม เนื่องจากมีบุคลากรจำนวนหนึ่งไม่ได้ถ่ายโอนมาด้วย สำหรับปัญหาสำคัญของระบบบริการสุขภาพปฐมภูมิ คือ คุณภาพของบริการยังต่ำกว่าเป้าหมายอีกมาก เนื่องจากงบประมาณและกำลังคนยังต่ำกว่าเกณฑ์มาตรฐาน ดังนั้นเพื่อแก้ปัญหานี้รัฐบาลควรพิจารณาพัฒนากำลังคนและงบประมาณให้เพียงพอแก่ความจำเป็น
