Abstract

Due to at least two problems: 1) the number of patients with chronic illness is increasing, and therefore, primary care service need to become the main focus of the national health services. 2) The poor attitude towards primary care, by both service providers and service recipients. The community health nursing instructors initiated a proactive primary health care service as a strategic intervention of the academic services. The process that causes change is 1) Implementing the 1A4C concept to the primary health care system 2) Promoting a positive attitude of nursing students toward the service through family-based care services of teaching and learning the teaching & learning.
3) Applying family nursing care 4) Promoting collaboration to create multidisciplinary teams.  The changes evaluation was found that 1) 62 stroke patients were received basic needed care at home 2) the expense on the health care is reduced, family income increases due to a reduction in off-work days of family members, 3) The other family members were received knowledge against preventable diseases, 4) There was a statistically significant difference in attitudes towards primary care services at lower.05 level among 234 students joined this project, 5)Family members had a significantly higher level of knowledge about health care system at lower .05 level.
