ปัจจัยที่มีความสัมพันธ์กับการตัดสินใจทำพินัยกรรมชีวิต ของประชาชนไทย: การศึกษาเชิงสำรวจในยุคสังคมสูงวัย

บทคัดย่อ
ประเทศไทยกำลังก้าวเข้าสู่สังคมสูงวัยอย่างรวดเร็ว ทำให้การวางแผนการดูแลสุขภาพล่วงหน้า เช่น การจัดทำพินัยกรรมชีวิตตามมาตรา 12 พระราชบัญญัติสุขภาพแห่งชาติ พ.ศ. 2550 มีความสำคัญมากขึ้น งานวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาปัจจัยด้านประชากรศาสตร์และทัศนคติที่สัมพันธ์กับการตัดสินใจทำพินัยกรรมชีวิตในประชาชนไทย โดยใช้ข้อมูลจากการสำรวจ “สถานการณ์เด็กเกิดน้อยและสังคมสูงอายุ ปี 2567” ดำเนินการโดยสถาบันวิจัยประชากรและสังคม มหาวิทยาลัยมหิดล กลุ่มตัวอย่างจำนวน 1,042 คน อายุ 18 ปีขึ้นไป เก็บข้อมูลผ่านแบบสอบถามออนไลน์ วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาและการทดสอบไคสแควร์ ผลการศึกษาพบว่า เพศสภาพ (p = 0.029) กลุ่มอายุ/เจเนอเรชัน (p = 0.005) ภูมิภาค (p = 0.002) และระดับการศึกษา (p < 0.001) มีความสัมพันธ์กับความคิดเห็นต่อพินัยกรรมชีวิตอย่างมีนัยสำคัญ ขณะที่สถานภาพสมรสและอาชีพไม่พบความสัมพันธ์ นอกจากนี้ ทัศนคติต่อภาวะเด็กเกิดน้อย นโยบายส่งเสริมการมีบุตร และเงินอุดหนุนแรกเกิด รวมถึงทัศนคติต่อสังคมสูงวัย ได้แก่ การปรับนิยามผู้สูงอายุเป็น 65 ปี การขยายอายุเกษียณ และแนวคิด aging in place ล้วนมีความสัมพันธ์กับความคิดเห็นต่อพินัยกรรมชีวิตอย่างมีนัยสำคัญ (p ≤ 0.001) โดยกลุ่มที่เห็นด้วยกับพินัยกรรมชีวิตมีสัดส่วนเห็นด้วยกับการปรับนิยามผู้สูงอายุเป็น 65 ปี สูงที่สุด (70.2%) ผลการศึกษาชี้ให้เห็นว่าการตัดสินใจเกี่ยวกับพินัยกรรมชีวิตเชื่อมโยงทั้งปัจจัยเชิงโครงสร้างประชากรและทัศนคติต่อสังคมสูงวัย ดังนั้น การส่งเสริมพินัยกรรมชีวิตควรเน้นควบคู่กับการพัฒนาความรอบรู้สุขภาพ และการสื่อสารนโยบายสังคมสูงวัย โดยเฉพาะในกลุ่มวัยแรงงานและวัยหนุ่มสาวให้มากขึ้น
